order that we may provide you with dental care of the highest standard,
within your budget, we hope that the following outline of Professional Fees
will prove helpful. ·
Initial Consultation o
Usually, this will be offered as a 30
minute appointment, to allow a thorough examination of all oral hard and soft
tissues. Normally this will include X-Ray examination of your mouth. During
this appointment, you will be given at least an outline of your dental needs.
Inclusive fee: £60.00. ·
Routine Treatment o All
treatment offered which does not include the services of a dental technician.
This may amount to the provision of fillings, all of which are "tooth
coloured", laser therapy, minor oral surgery, etc. Such treatment is
provided on a time-taken basis. Inclusive fee: £240.00 per hour, pro rata for parts thereof. ·
Specific Treatment o
Items
of treatment, involving the services of a dental technician, for which a
fixed fee, inclusive of surgery and laboratory time and all materials would
be charged. Examples:
o
Major reconstructive dental treatment:
This would include multi-unit fixed bridgework, dental implant-supported
prostheses and other complex restorative treatment plans. Such undertakings
will involve the drafting of a report and detailed Statement of Fees,
together with a dedicated, written Consent Form. ·
Dental Hygienist. o Both
routine and specific dental hygiene treatment is provided on a time-base, at
5 minute intervals. Normally, a 30 minute appointment is scheduled. Inclusive fee: £84.00 per hour,
pro rata for parts thereof. ·
Payment of Professional Fees. o You
will be advised of inclusive fees at the commencement of treatment. Accounts
are due for settlement at the final visit of a course of treatment, or in
accordance with an agreed deposit/staged payments protocol. We accept payment
by cash, cheque, Switch and all major credit cards. For

or via telephone or postal references listed above. Cancellation Charges: Problems will always occur and the Practice is very sympathetic to the
occasional extreme circumstances, whereby appointments cannot be kept. Goodwill
can be maintained if every effort is made to attend for dental appointments,
but where persistent non-attendance occurs, or an appointment is cancelled
where it would be impossible to fill the time lost, the following charges
shall apply: ·
Unexplained non-attendance or non-extreme
cancellation of an appointment with the
dentist: £60.00 per hour, or parts thereof. ·
Unexplained non-attendance or non-extreme
cancellation of an appointment with the
hygienist: £30.00 per hour, or parts thereof. Complaints
Procedure: We
strive to provide quality dental care appropriate to your needs, in a
pleasant and stress free environment. Occasions do occur where there may be
concerns or complaints about the way you are treated. We
hope that the following complaints procedure will help you. Full
confidentiality is assured in bringing your complaint to our attention. Point of
contact personnel in first instance: Your concerns can be
advised verbally and in confidence to Dr Steven Parker, Mrs. Penny Parker or Mrs. Pauline Parker
(Practice Manager) Formal Complaint: These should be submitted
in writing to Dr Steven Parker and will receive a written response. Serious Formal
Complaint: These can be addressed in
writing to Dr Steven Parker. Data Protection: Parkers Dental Practice is registered under
the Data Protection Act 1998. In very limited circumstances or when required
by law or a court order, personal data may have to be disclosed to a third
party not connected with your health care. In all other situations, disclosure
will only occur when we have your specific consent. Where possible, you
will be informed of these requests for disclosure. Additional
